Submit this form to cancel your Uptimia subscription. You can also cancel from inside your account or by emailing admin@uptimia.com.

Use the email address associated with your Uptimia account if possible.

Optional. Helps us identify your contract if the email above is not on file.

Optional. Found in your invoices or in your account settings.

For example: "Uptimia Pro monthly subscription".

Optional. Leave blank for the next available cancellation date.

Optional for ordinary cancellation; required if you select extraordinary cancellation.

We will send a confirmation email to the address you provide immediately after submission, in line with § 312k (2) BGB. The cancellation takes effect on the date stated above; if you did not specify a date, an ordinary cancellation takes effect at the end of the current billing period.
